If you have question regarding payment and insurance information, please dont' hesitate to call us during regular office hours. If you call after hours, you may leave a message, we will return your call the next business day.

 
Business and Payment Policies
Mesa Pediatrics is contracted with a number of Insurance Plans. If we are contracted with your plan, we will bill the insurance company for the physician services we provide in the hospital or in the office. If we are not contracted with your plan, we cannot bill for you and payment is expected at the time of service.

It is necessary that you bring your child's insurance card with you to every visit. We make a copy of the card at each visit. The copy is used to submit an insurance claim for the visit. We will need another copy at the next visit, to go along with the next insurance claim.

It is very important that the insurance card be a current copy and that the card specifically list the name of the child that is being seen in the office. If you have recently enrolled in a new insurance plan, you may bring a copy of your enrollment form until you receive the new card.

For newborns, we are able to accept a copy of the parent's insurance card up until the four-month visit. After that time, we must receive a copy of the child's card.

If you are unable to present a current insurance card for your child, you will be asked to sign an Insurance Coverage Waiver form.

In addition, it is very important that you fill out a new Patient Information form for each child every six months. If you would like, you may print this form and fill it out in advance: Click Here for the Patient Information Form.

Mesa Pediatrics collects all patient co-payments at the time of service. If a patient balance is generated after a claim is submitted to a contracted insurance plan, we expect payment in full after a bill is generated. Patients who fail to pay their balance in full will be subjected to collection procedures.
